• Virginia Health Form (filled out by child's physician).
• Virginia Immunization Exemption Form (if applicable). See the Virginia Immunization Requirements for details.
  
• Signed Illness Pick-Up Agreement
  
• Signed Handbook Policy Acknowledgement Form
  
• Tuition payment
